CHOICES is a safe, welcoming clinic that offers comprehensive reproductive health care to everyone.

Whether you’re here for birth control, abortion, prenatal care, STI testing, or any other service, we are ready to help. Our goal is to help you make informed choices about your own health. We’re dedicated to making sure everything is as easy and comfortable for you as possible. We keep your information 100% confidential, and we seek donations in order to offer a Patient Assistance Fund for those who need help paying.

Plan Your Visit

  • Identification. We will need to see an ID with a photo and date of birth. We need to verify your age for certain services.
    • If you are under 18 years old, we will accept your social security card, health insurance card, school ID, or your recent report card as identification.
  • Your payment in the form of a money order, VISA, or Mastercard. We cannot accept checks or cash for security purposes.
  • Insurance cards if you are using your insurance for services. Please call your insurance carrier for details about your own coverage and benefits. Please call to verify that we can accept your specific plan before your visit.
  • Patients who choose to receive moderate sedation are required to have a designated driver present prior to the procedure.

Yes, please feel free to have a healthy balanced meal before your appointment.

Appointment times vary according to the type of services received.  For Wellness Appointments please plan to be with us for about an hour and a half. For abortion appointments, please plan to be at the clinic for up to 5 hours. You may want to bring something to read, as well as a snack to eat while you wait.

For abortion appointments: Only visitors accompanying  patients who choose to take an optional one time, pre-procedural dose of anti-anxiety medication, minors, and support persons for individuals with disabilities.

NOTE: Patients who choose to receive a one time, pre-procedural dose of anti-anxiety medication ARE required to have a designated driver present prior to the procedure.

For wellness/other appointments: Only visitors accompanying minors, support persons for individuals
with disabilities, and children ages 13 years and younger (if unable to arrange childcare).

For midwifery (prenatal or postpartum) appointments: you may bring one healthy guest, plus children aged 13 and younger if unable to arrange childcare. 

For labor and birth: you may bring two healthy guests, plus one doula if you have one.

Patients and visitors should check in at the front desk upon arrival. All patients and visitors must wear a mask fully covering the mouth and nose for the duration of their visit. 

At our Memphis clinic, our parking lot entrance is on Bellevue Boulevard, and the lot is behind the building. Security will check you in at the main entrance, which is in the back of the building. The doors facing Poplar Avenue remain locked at all times.

At our Carbondale clinic, our parking lot is directly in front of the building on Giant City Road. Security will check you in inside the building. The doors facing Giant City Road remain locked at all times.

For Abortion Appointments

Parental involvement is not required in Illinois. If you're younger than 18, you can consent to an abortion and do not have to notify your parents.

For Other Appointments
In the state of Tennessee, anyone over the age of 13 can consent to reproductive health exams, birth control, and STI testing without parental consent. We encourage you to discuss your reproductive health issues and/or your plans for contraception with your parents, and they are welcome to attend your reproductive health visit with you.  If there is some reason you don’t believe their presence or involvement would be in your best interest, however, you can access reproductive health care on your own.

Our Carbondale, IL clinic is now seeing patients. Please call us to make an abortion appointment. Abortion services are not available in Memphis due to a law banning abortions entirely in the state of Tennessee. Visit our Abortions page to learn more.
